HOW TO APPLY
Application Overview and Rules
Agape Orthodontics offers two separate orthodontic scholarship programs for students seeking orthodontic treatment. Each program has different eligibility and application requirements.
Applicants may apply to only ONE program per year.
IIMPORTANT NOTICES (Read Before Applying)
- Only one application permitted per applicant.
Submitting applications for both programs will result in automatic disqualification. - Application deadlines apply.
Late or incomplete applications will not be reviewed. - Incomplete applications will not be considered.
All required documents must be submitted at the time of application. - Awards are not transferable.
The selected recipient must begin treatment within the specified timeframe. - Failure to comply with treatment requirements may result in forfeiture.

Review both programs carefully. Once submitted, applications cannot be transferred between programs.
Purpose
This award is designed for students whose families face financial barriers that make treatment otherwise unattainable.
Eligibility
- Ages 13–19
- Demonstrated financial need
- Willing to commit to orthodontic treatment
Required Documents
All applicants must submit every item below.
- Completed Application Form
- Most Recent Transcript
- Two positive letters of recommendation
- Proof of Financial Need(at least one required)
What We Recognize
This award recognizes students who demonstrate strong character, leadership, academic effort, or community involvement.
Ideal Candidates
- Ages 13–19
- Demonstrated leadership, service, and/or academic effort
- Willing to commit to orthodontic treatment
